Therapie
Indikation
>18 J.: mittelschwere bis schwere Plaque-Psoriasis, aktive Psoriasis-Arthritis als Monotherapie oder in Kombination mit Methotrexat nach ungenügendem Ansprechen oder Unverträglichkeit auf DMARD; mittelschwere bis schwere aktive Colitis ulcerosa oder mittelschwerer bis schwerer aktiver Morbus Crohn bei ungenügendem Ansprechen, Ansprechverlust oder Unverträglichkeit auf eine konventionelle Therapie oder ein Biologikum.
Dosierung
Fertigpen/-spritze: s.c. Inj. (Abdomen oder Oberschenkel); Inf. Konz.: i.v. Inf. (mit gering proteinbindendem In-Line Filter 0,2 µm) nach Verdünnung in NaCl 0,9% auf 250 ml (Endkonzentration 0,8 mg/ml).
Plaque-Psoriasis, Psoriasis-Arthritis
>18 J.: 100 mg als s.c. Inj. in den Wo. 0 und 4, dann alle 8 Wo.
Colitis ulcerosa
Induktion: >18 J.: 200 mg als i.v. Inf. über min. 1 h in Wo. 0, 4 und 8.
Erhalt: >18 J.: 100 mg als s.c. Inj. in Wo. 16, dann alle 8 Wo.; alternativ evtl. 200 mg als s.c. Inj. in Wo. 12 und dann alle 4 Wo.
Morbus Crohn
Induktion: >18 J.: 200 mg als i.v. Inf. über min. 1 h in Wo. 0, 4 und 8 oder 400 mg als s.c. Inj. in Wo. 0, 4 und 8.
Erhalt: >18 J.: 100 mg als s.c. Inj. in Wo. 16, dann alle 8 Wo.; alternativ evtl. 200 mg als s.c. Inj. in Wo. 12 und dann alle 4 Wo.

Kontraindikation
Klinisch relevante aktive Infektionen wie aktive Tuberkulose. Schwangerschaft (bis min. 12 Wo. nach Therapie), Stillzeit.
