Therapie
Antithrombotika > Faktor-Xa-Inhibitoren > Direkte Faktor-Xa-Inhibitoren
Indikation
Therapie venöser Thromboembolien (VTE) nach initialer parenteraler Antikoagulation zur Prophylaxe rezidivierender VTE bis 18 J., Thromboseprophylaxe nach Fontan-Eingriff bei Patienten von 2–18 J. und ≥10 kg.
Dosierung
Mit dem Essen; Gabe über Magensonde möglich.
Therapie und Rezidivprophylaxe von VTE
≥4 kg: Granulat zu 5,25 g; <4 kg: Granulat zu 2,625 g; initial parenterale Antikoagulation während min. 5 Tagen; Therapiedauer: 3–max. 12 Mon. (<2 J. mit Katheter-assozierter VTE: 1–max. 3 Mon.).
<18 J. und ≥50 kg: 1×tgl. 20 mg.
<18 J. und 30–50 kg: 1×tgl. 15 mg.
<18 J. und 12–30 kg: 2×tgl. 5 mg.
<18 J. und 2,6–12 kg: 3×tgl. «FI».
Thromboseprophylaxe nach Fontan-Eingriff
≥30 kg: Granulat zu 5,25 g; <30 kg: Granulat zu 2,625 g; Therapiedauer «FI».
2–18 J. und ≥50 kg: 1×tgl. 10 mg.
2–18 J. und 30–50 kg: 1×tgl. 7,5 mg.
2–18 J. und 20–30 kg: 2×tgl. 2,5 mg.
2–18 J. und 12–20 kg: 2×tgl. 2 mg.
2–18 J. und 10–12 kg: 2×tgl. 1,7 mg.

Kontraindikation
Akute bakterielle Endokarditis, klinisch signifikante aktive Blutung, schwere Lebererkrankung mit Koagulopathie, Leberinsuffizienz mit Koagulopathie, dialysepflichtige Niereninsuffizienz, gastrointestinale Ulzera. Schwangerschaft, Stillzeit.
