>18 ans: 1 g 3×/j. i.m./i.v. ou 2–8 g/j. en 3–4 perf. (max. 2 g/perf.); endocardite à staphylocoques résistant à la pénicilline G: 12 g/j. avec un aminoside.
Enfants: jusqu'à 150 mg/kg/j. i.m./i.v. (dose unitaire max. 33 mg/kg).
