Indications
Carcinome rénal avancé en monothérapie après un traitement antérieur par anti-VEGF, carcinome rénal avancé (non résécable ou métastatique) en traitement de première ligne en association au nivolumab lors d'un profil de risque intermédiaire/défavorable, carcinome hépatocellulaire en monothérapie après un traitement antérieur avec sorafénib; carcinome thyroïdien différencié, localement avancé ou métastatique, en monothérapie chez des patients réfractaires ou non éligibles à l'iode radioactif avec progression pendant ou après un traitement par anti-VEGF.
Posologie
Min. 1 h avant ou 2 h après un repas.
Monothérapie: >18 ans: 60 mg 1×/j.
En association au nivolumab: >18 ans: 40 mg 1×/j. (nivolumab: 240 mg toutes les 2 sem. en perf. i.v. pendant jusqu'à 24 mois).
Contre-indications
Procréation chez l'homme et la femme (jusqu'à min. 4 mois après le traitement), grossesse «IPr», allaitement (jusqu'à min. 4 mois après le traitement).

LIMITATION
Pour le traitement des patients adultes atteints d’un carcinome des cellules rénales (renal cell carcinoma, RCC) avancé après une thérapie antérieure ciblée des récepteurs du facteur de croissance de l’endothélium vasculaire (VEGF). La posologie maximale est de 60 mg par jour. Les patients doivent être traités jusqu’à la progression de la maladie ou jusqu’à l’apparition d’une toxicité intolérable. Les patients cliniquement stables après la découverte initiale d’une progression peuvent continuer à être traités jusqu’à la confirmation de la progression de la maladie. Le traitement nécessite l’accord sur la prise en charge des frais par l’assureur-maladie après consultation préalable du médecin-conseil.
