Indications

Prévention et traitement des maladies thromboemboliques (y c. infarctus du myocarde), coagulation intravasculaire disséminée, anticoagulation du circuit extracorporel (machine coeur-poumons, hémodialyse), prévention et traitement de l'hyperlipidémie.

Posologie

Inj. s.c., inj. i.v. ou perf. i.v.
Prévention de la thromboembolie
Chirurgie, médecine interne: 5000 UI s.c. (chirurgie: 1ère dose 2 h avant l'intervention), puis 5000 UI toutes les (8–)12 h pendant min. 7 j. ou jusqu'à mobilisation complète.
Risque élevé de thromboembolie: mêmes schémas que pour le traitement, début: 2–4 j. après l'opération.
Maladies thromboemboliques
Perf. i.v. (1er choix): au début 5000–10'000 UI en inj. i.v., puis 20'000–30'000 UI/j. en perf. i.v., puis selon aPTT.
Inj. i.v.: 40'000–50'000 UI/j. en 4–6 inj., puis selon aPTT.
Inj. s.c.: au début 5000–10'000 UI en inj. i.v., puis 40'000–50'000 UI/j. en 2 inj., puis selon aPTT.
Enfants >2 mois: au début 50 UI/kg en bolus i.v., puis 20 UI/kg/h en perf. i.v. continue, puis selon aPTT, alternative: dose journalière en 2–3 inj. s.c.
Après thrombolyse: perf. i.v.: 20'000 UI/j., puis selon aPTT.
Autres indications: «IPr».

Contre-indications

Thrombopénie induite par l'héparine, diathèse hémorragique, hépatopathie sévère, néphropathie sévère, pancréatopathie sévère, ulcère gastro-duodénal, tumeur gastro-intestinale, hypertension (diastolique >105 mm Hg), hémorragie cérébrale, traumatisme du SNC, anévrisme cérébral artériel, intervention chirurgicale du SNC ou de l'oeil, rétinopathie, hémorragie du corps vitré, endocardite bactérienne subaiguë, avortement imminent, anesthésie rachidienne, anesthésie péridurale, ponction lombaire; grossesse «IPr»; nouveau-nés et prématurés (25'000 UI/5 ml).
