Standarddosis: Erw., Jugendl., Kinder: 2×/Wo. 15–20 mg/m2 als i.v. Bolus; 1×/Wo. 30–50 mg/m2 als i.v. Bolus; 15 mg/m2 als i.v. Bolus oder i.m. während 5 Tagen alle 2–3 Wo.
Mittlere Dosis: Erw., Jugendl., Kinder: 150 mg/m2 als i.v. Bolus alle 2–3 Wo.; 240 mg/m2 als i.v. Inf. über 20 Min. alle 4–7 Tage; 500–1000 mg/m2 als i.v. Inf. über 36–48 h alle 2–3 Wo.
Hochdosis: Erw., Jugendl., Kinder: 1000–12000 mg/m2 als i.v. Inf. über 1–24 h alle 1–3 Wo.
Intrathekal: «FI».
